There are two ways to establish an account and place an order:
To be invoiced:
- Fill out this credit application (Word format) and email to jminicola @ capella.ca (remove spaces)
- OR Fill out this credit application (PDF format) and fax to Jeff Minicola at 705-748-4535
- Once your application is processed, contact inside sales at 705-748-3255 and place your order.
To order with credit card: 
- To order with VISA, Mastercard, or American Express, contact inside sales at 705-748-3255 and place your order. Please note that for credit card orders, a surcharge of 3% will be applied.
